What is a Family Health Group (FHG)?
A
FHG is a group of three or more physicians who contract with the Ministry of
Health and Long Term Care (MOHLTC) to provide a 24 hour, 7 day a week family
practice service to a group of Rostered Patients.
Patients agree to roster with a family physician within a FHG and see a
member of that group for most or all of their family practice needs, in
return for the 24/7 coverage.
Rostering requires a patient to sign a
MOHLTC form stating that they have rostered with a member of the FHG.
Parents or guardians sign on behalf of their children or wards.
When you roster at the Abbey Medical
Centre with Dr. Kerr, Dr Guirguis, Dr Sayed , you automatically become part of the North
Oakville FHG.
Please read the guidelines from the
MOHLTC when you are asked to roster.
