effective September 1st. The average cost of file transfer is $30.
During September, the office has not been able to
find a replacement for Dr Alam, therefore rostered patients may use the
offices of the other FHG members

Remember all rostered patients are members of the North Oakville FHG
and whenever doctors from Abbey Medical are unavailable, rostered
patients may be seen at one of the other locations (see Links)

follows:
9am to 11:30am & 4pm to 6:30pm
There will be a $10 prescription renewal fee
Request for copies of results will be $5.
Complete Physical Appointments

There will be a $75.00 fee for missed Physical appointments. All patients must
give 24hr notice for cancellations of these appointments. |
